Program details change often. If a record on this site is wrong, incomplete, or out of date, tell us what is inaccurate and where we can confirm the correct information.
What to include
- The program name and the page where the record appears
- The specific field that is inaccurate
- The correct information, stated plainly
- A source we can check, such as a licensing record or the program's own page
- Your name and role, so we can follow up if something is unclear
Do not include patient information
How we handle requests
We check the request against the sources we accept, update the record where the correction is supported, and change the last-verified date. Where a claim cannot be confirmed, we leave the field blank rather than publishing an unverified statement.

Who can request a correction?
Anyone. Requests from programs and from the public are handled the same way, and each is checked against sources before a record changes.
Will a correction change where a program appears?
No. Corrections update the accuracy of a record. They do not affect placement, and placement is not for sale.
What if a program wants a listing removed?
Send the request with the program name and the reason. We will review it and respond, and we record the date of any change.
